About the application:

Tactio Care is a health coaching tool offered through your employer, health plan or health care provider. The Tactio Patient apk, accessible on iOS and Android device, is your companion apk throughout the tool, supporting you with the following: CARE PLAN A personalized care plan will be made by your health care provider. The plan will contain measurements (ex: take blood pressure everyday), questionnaires (ex: answer to wellness check monthly) or another instructions (ex: check your feet weekly). The apk will remind you when it’s time to act with a push notification, and by showing active “Tasks” on the home screen. All you have to do is tap and follow the on-screen instructions. Tasks typically take a min or less. RESULTS The details collected in the apk will be created accessible to your health provider. You can also see the info in the “Results” section, which uses graphs and color codes to add meaning to your data. MESSAGING WITH YOUR PROVIDER You will also be able to exchange messages (text, image) with your health provider using the “Inbox” section of the app. N.B. Tactio Patient is invitation-only. If you would like to participate, ask your employer, health plan or health care provider if they sponsor the tool.
